Home-based interventions
What is it?
"Life Visits" are free home and community-based services that complement the social services of municipalities.
Life Visits are an award-winning innovative practice at local, national and European level.
The interdisciplinary team of Life Visits consists of a geriatrician, health supervisor, social worker, nurse, psychologist, music therapist, legal and administrative staff, and provides home-based interventions and activities in the community.
Home interventions
The Co2gether mobile unit, following a referral, carries out an on-site needs assessment and co-ordinates with social services (e.g. home help, Roma community centres) the type of actions to be provided.
The personalised approach includes meeting food needs, providing health materials and psychosocial support services as well as home improvements or the purchase of household equipment.
Cumulative criteria for inclusion in Life Visits:
- people of all ages living on the edge of poverty or experiencing socio-economic exclusion
- persons residing in remote or mountainous areas of the Region of Western Greece and specifically in 3 prefectures (South Aitoloakarnania, South Achaia, South Ilia) and in 8 municipalities out of the 19 municipalities of the Region of Western Greece.
Interventions in the community
At the request of the social services of the cooperating municipalities, empowerment, health prevention and information actions are implemented in the community (e.g. in detention centres, KAPI, community centres, schools, universities).
The aim of the Life Visits is to improve the quality of life, mental resilience and the smooth integration of the beneficiaries into society. It is based on thorough assessment, identification and response to actual needs rather than offering a list of services to which the beneficiary can apply to receive.

- Ergonomic changes and construction improvements to the home (e.g. water heaters, cookers, solar energy panels, roof and floor repairs, painting, beds, clothing, bedding)
- Advice on hygiene and safety at home
- Family counselling and referral to special services for the management of children, adults with disabilities
- Practical care exercises for carers with modern technological equipment (serious game, VR)
